During a patient visit at NorthShore Health Centers, a Family Practice doctor or nurse practitioner will review your medical history, vital signs and other important readings and information collected by our professional nursing staff. If necessary, further tests will be ordered to determine a diagnosis. NorthShore Health Centers’ doctors and nurse practitioners – and when needed, a behavioral health consultant – will then create a treatment plan which may include medication, dietary changes, minor surgical procedures, or other care as needed. A follow up visit will also be scheduled. You can rest easy knowing that our state-of-the-art technology makes vital patient information instantly available to our providers at the time of your appointment.

Concerned about Diabetes?

Diabetes is a serious disease that can cause heart attack, stroke, blindness, kidney failure, or loss of feet or legs.
