NorthShore Health Centers has opened a school-based health center in collaboration with the La Porte Community School Corporation in order to provide primary and preventative care as a benefit for LPCSC employees and their families in order to address the needs and improve the health of the La Porte Community.

- Illness Visits (Flu, colds, aches, pains)
- Monitor ongoing health concerns (Diabetes, Asthma, Heart Disease, High Blood Pressure)
- Preventative Care
- Case Management
- Wellness Counseling
- Pharmacy
- Transfer Prescriptions
NorthShore Health Centers clinic, located inside the LaPorte High School, provides Family Nurse Practitioner offering medical care, minor lab testing and medications as required to treat the La Porte Community School Corporation who may need these services. If a patient needs a prescription, the Nurse Practitioner can electronically prescribe the medication to the La Porte NorthShore Pharmacy located at 504 Legacy Plaza West or participating 340B pharmacy for quick pharmacy service.
